Brian Denis Cox 1946. június elsején született a skót Dundee-ben. Ír felmenőkkel rendelkezik; az öt testvér közül ő a legfiatalabb. Szülei kétkezi munkások voltak: édesanyja egy malomban dolgozott, s számos idegösszeroppanást kapott, míg édesapja hentesként, majd cipészként dolgozott. 14 évesen csatlakozott a Dundee Repertory Theatre társulatához, majd ezt követően egy drámaiskolába felvételizett. Parádés szereposztásban játszott a Rob Roy (1995) című történelmi filmben - Liam Neeson, Jessica Lange, John Hurt és Tim Roth oldalán mutatkozott, majd Jeremiah Cassidyként találkozhattunk vele Andy Garcia és Michael Keaton társaságában a Gyilkos donor (1998) című akciófilmben. Szintén a főhősök sorában kapott helyet a Sinatra árnyékában (2001) és A királyné nyakéke (2001) című drámákban, majd a Bújj, bújj, szellem! (2003) című thrillerben díszlettervezőként, míg a The Dead One (2007) horrornál rendezőként és forgatókönyvíróként volt jelen. Az elmúlt években Ivan Simanov szerepében Helen Mirren forrófejú orosz szerelmét formálta meg a Red (2011) és a Red 2 (2013) című akciófilmekben.
